Select a chronic disease/condition that will be the focus of the concept map that you will be creating. The disease/condition can be one that has been discussed in the course thus far or one that interests you.
Chronic disease: Cystic Fibrosis
Decide how you will create your concept map. The map can be created by hand or by using a computer program. There are some mind-mapping programs that are fantastic and have free trials. PowerPoint has functions to create mapping, also. You must be able to submit your work to BlackBoard, so that is an important aspect of your decision.
The map must contain the following concepts:
- A basic review of the pathophysiology
- Expected treatments including common medications that are prescribed

- At least four chronic illness concepts applicable to this chronic condition
- At least three nursing interventions for each concept
- An evaluation plan
- Psychosocial issues that may be present
- Interprofessional team members
Clear connections between the concepts need to be indicated. A connection line should be drawn between the two. An example might be that you indicate that an individual suffering from the condition may be prone to feeling depressed or hopeless.
